Correct treatment depends on identifying whether the cause is nasal allergy, sinusitis, AC-related irritation, turbinate swelling, deviated nasal septum, nasal polyps or another condition.

Why the Same Treatment Does Not Work for Everyone
Two patients may both complain of blocked nose, but the reason may be completely different. One may have allergic rhinitis, another may have sinusitis, turbinate enlargement, a deviated nasal septum or AC-related dryness.
Common causes include:
- Nasal allergy or allergic rhinitis
- Acute or chronic sinusitis
- Deviated nasal septum
- Turbinate swelling
- Nasal polyps
- AC-related dryness and irritation
- Recurrent infections
- Snoring or mouth breathing
- Eustachian tube dysfunction causing ear fullness
This is why treatment should be diagnosis-based rather than repeated trial and error.
Step 1: Identify the Cause
Before starting treatment, your ENT doctor will assess the pattern of symptoms, triggers, previous medication use, allergy history, sleep symptoms and associated ear or throat complaints.
A nose, throat and ear examination is done. If symptoms are persistent or recurring, nasal endoscopy, a brief in-clinic procedure using a thin camera to examine the inside of the nose, may be recommended. A CT scan is not required for every patient and is usually advised only when sinus disease is suspected, symptoms are recurrent, or surgery is being considered.
Saline Nasal Care
Saline nasal spray or saline rinses can help with nasal dryness, allergy symptoms and postnasal drip. They may moisturise the nasal lining, clear dust and allergens, reduce crusting and improve comfort in air-conditioned environments.
Saline care is useful, but it may not be enough if symptoms are severe, one-sided, recurrent, associated with facial pain, or linked to reduced smell or ear blockage.
Allergy Treatment
If nasal allergy is the main cause, treatment focuses on reducing inflammation and controlling triggers. This may include dust reduction, cleaning AC filters, avoiding smoke and strong fragrances, antihistamines when needed, saline nasal care and nasal steroid sprays if prescribed.
Nasal steroid sprays can be effective when used correctly, but technique matters. Spraying toward the septum or using them irregularly may reduce benefit or cause irritation. Allergy treatment is usually about control and prevention, not only short-term relief.
Sinusitis Treatment
Sinusitis treatment depends on whether it is acute, recurrent or chronic. Management may include saline irrigation, nasal sprays, pain and fever control, allergy treatment if it is contributing, and antibiotics only when clinically indicated.
Not every sinus symptom needs antibiotics. Facial pressure, headache and blocked nose can also occur due to allergy, nasal inflammation, migraine or structural nasal blockage. If symptoms keep coming back, evaluation helps identify why.
Treatment for AC-Related Dryness
If symptoms are mainly related to AC exposure, environmental correction is important. Avoid direct AC airflow on the face, keep the temperature moderate, clean filters regularly, stay hydrated and use saline spray if the nose feels dry.
If symptoms continue despite these steps, AC may not be the only cause. Allergy, reflux, nasal blockage or snoring-related mouth breathing may also be contributing.
When Is Surgery Needed?
Most patients with blocked nose or sinus symptoms do not need surgery. Surgery is considered only after proper evaluation and when medical treatment or environmental measures are not enough.
Surgery may be discussed when there is chronic sinusitis not responding to appropriate treatment, recurrent sinus infections, nasal polyps, significant deviated nasal septum, persistent turbinate enlargement or structural blockage confirmed on evaluation.
The aim of surgery, when needed, is to improve drainage, airflow or access for treatment, not to operate on every patient with a blocked nose.
Why Overusing Nasal Drops Can Be Harmful
Some decongestant nasal drops give quick relief, but overuse can worsen nasal blockage over time. Patients may feel they cannot sleep without the drops, and the nose blocks again soon after the effect wears off.
If you need nasal drops repeatedly, it is better to seek an ENT evaluation. The underlying problem may be allergy, turbinate swelling, deviated septum or another cause that needs proper treatment.
